HEALTH RECORDS & IMMUNIZATIONS
As per section 65C-22.006(2)(a,b,c) of the Florida Department of Children and Families Rules and Regulations the following forms are required to verify that your child has received the proper health exam and immunizations.
These forms are due prior to school starting. HRS Physical Health Form #680 is valid for 2 years after the most current physical date.
PLEASE NOTE: When parents have opted out of having their children vaccinated, we can accept an exemption form provided and signed by your pediatrician in lieu of Form #3040 listed above. In either case, state law compels us to have the Physical Health Form #680 as a part of your child's preschool file. The exemption form is available at the local health department.
